Our membership fee is $600 per year, which is not covered by insurance.
We are providers (in-network) ONLY with the TWO following insurance companies.
- Medicare (Original/Traditional)
- Tricare (Prime, Standard & For Life)
- We do not accept or bill PPOs, Medi-Cal or HMOs.
- Payment is due at the time of service (copays, deductibles, and any out of pocket).
In order for our office to verify and check benefits, we will need copies of your insurance cards.
