Insurance and Payment Information

We accept Check, Cash, Visa or MasterCard.

We will bill your insurance company, however we may ask for payment at the time of service if we are not providers with your insurance company. However, we do not accept or bill Medi-cal or HMOs. We are providers (in-network) with the following insurance companies.

Please call your insurance company or our office for additional questions about your insurance coverage.  In order for our office to verify and check benefits, we will need copies of your insurance cards.
